With the passage of the Therapeutic Shoe Bill, At-Risk Diabetic may qualify for 1 Pair of off-the-shelf depth shoes and 3 pair of heat moldable muti density inserts or 1 Pair off-the-shelf depth shoes and 3 pair custom molded inserts.

Eligibility of Coverage under Therapeutic Shoe Bill requires:

  • Patient is being treated under a comprehensive plan for Diabetes Management
  • One or more of the following indicators are present
  • Complete or partial amputation of the foot
  • History of previous pre-ulcerative callus
  • Peripheral Neuropathy w/ evidence of callus formation
  • Foot Deformity
  • Poor Circulation
  • Patient requires diabetic shoes and inserts.

  • Statement of Certifying Physician
  • Diabetic Management Notes
  • Foot Exam Notes

  • Please bring your picture ID and Medicare/Insurance cards

We are certified Medicare Provider and In network provider for Independence Blue Cross and Keystone Health Plans.
